Pharmacist's home made cough syrup. Is this a funky formula?

I would be most worry about the mive overdose of the lethal neurotoxin aconitine (the aconite tincture), which occurs at around 1.5 - 6mg. Unless 1199mg of that tincture is something else, I would request it be removed.
